What goes on it
Name, grade level, school year and term. The subjects you actually taught, not a standard list you feel you ought to be able to show. One mark per subject in one system, with that system stated on the card. Days of instruction if your state counts them. A short comment line and a signature. That is the whole document, and anything beyond it is decoration.
Why the average sometimes does not appear
Two reasons, both deliberate. A mastery or narrative record produces no number at all, because reducing “secure” and “working towards” to a decimal invents precision that was never there. And if a single mark cannot be read against the system you picked, the average is withheld entirely rather than computed across the rest: a partial average looks authoritative and is not. The card names the subject so you can fix it.
A report card is not a transcript
A report card is a snapshot of one term, mostly for you, your child, and occasionally a grandparent or a co-op. A transcript is the cumulative high school record a college reads. They are different documents with different jobs, and a stack of report cards is not a substitute for the second one.
